We have recently moved positively forward within a new Locality-based structure and now have three Locality operational teams working in the East, North and South of the Borough delivering support to older people and adults with physical disabilities.
Each Locality team has its own identity arising out of the demography of their distinct geographical areas. Slough Borough Council has a vision to enable the teams to be based within the local area which they each serve in order to build strong links with community organisations and other professional agencies within their areas. Moves out of the central Council building and into locally based offices are planned for early 2019.
The focus of the work in the Locality teams is on strengths-based approaches and we work closely with our Reablement and Long term Occupational Therapy teams to promote independence and wellbeing wherever possible. We also work alongside a local voluntary agency whose role is to link people with community groups who can provide early support to help maintain independence and reduce isolation.
Where long term care and support needs are identified, we believe in excellent, positive, evidence-based practice to deliver Care Act assessments and care and support plans for individuals with eligible needs. The teams are well supported to deliver creative and personalised care and support plans through close working relationships with our Purchasing and Personal Budget Team and we actively promote Direct Payments and support for Carers.
Each team runs their own Duty service and safeguarding enquiries as well as reviewing activities, so that the range of work is very varied and provides many learning opportunities to develop new skills and build positively on existing work experiences.
You will be working at the forefront of our vibrant Adults and Communities Directorate and will be vital to delivering the council’s responsibilities for Slough residents within the Care Act in a highly diverse, entrepreneurial and enabling community.
