The Hospital Social Work Team (HSWT)

The HSWT works with residents who have been admitted to any hospital. They work across the local acute hospital (Wexham Park), the community hospitals (Upton and St. Marks Hospitals). They also cover out of Borough referrals which can include Frimley Park, Royal Berkshire Hospital and other London Hospitals.   The team provides;

- Advice and information, including sign posting
- Conduct strengths based conversations to make the most of people assets and maintain independence.
- Discuss the onwards referrals to other services e.g. Reablement or Long–term O.T
- They provide support where patients have an ongoing need for a care package or where alternatives, to going home now need to be considered.
- The HSWT follow up the patient on the day after and six weeks after discharge.

All of the Boroughs social work teams with strength based approaches and use community resources, to maximise an individual’s abilities in meeting their outcomes. The HSWT consist of both Social workers and Social work assistants. They are based in the IRIS office, at Wexham Park, Slough, where the integrated approach means working directly with the Discharge team, the GP specialising in Discharge, Berkshire health care Foundation Trust and other local Social work teams.
